Data controller
The controller of data collected through this website is VDV clinic s.r.o., company ID (IČO) 09183795, with its registered office at Na výsledku II 1025/8, Nusle, 140 00 Praha 4. For personal data enquiries, contact info@vdv.clinic.
What data we collect
When requesting an appointment, you provide your name, phone number, email and chosen clinic. You may also select an area of care and add a note. We record the page the request came from, submission time, consent confirmation and processing status.
Do not include personal identification numbers, medical reports or a detailed medical history in the note. Discuss health concerns and records directly with a healthcare professional. This form is for arranging a visit, not maintaining medical records.
How we use your data
We use the data to respond to your request, pass it to the relevant clinic and arrange an appointment. The form includes consent to process your contact details; you can withdraw it by emailing info@vdv.clinic. We do not add your contacts to marketing lists. Submitting the form does not itself confirm an appointment.
Who can access your data
Your request is stored on the server in the website administration. Authorised staff and people providing technical operation have access. The request content is also sent by a bot to a closed work group in Telegram so the team can respond.
Telegram is an external cloud service. Group messages are not end-to-end encrypted secret chats. Using the service may involve processing outside the European Economic Area. Telegram's privacy policy explains how its service operates.
How requests are stored
Requests in the administration and their Telegram copies are kept without a predetermined end date. Automatic deletion is not configured. This does not affect your right to request erasure, restrict processing or withdraw consent. We assess such requests under applicable law.
Your rights
Subject to the conditions of the GDPR, you may request access, correction, erasure, restriction of processing or data portability. You may withdraw consent at any time; withdrawal does not affect the lawfulness of earlier processing. To exercise your rights, email info@vdv.clinic. We will verify your identity in a way proportionate to the request.
You may lodge a complaint with the Czech Office for Personal Data Protection (Úřad pro ochranu osobních údajů). We do not use website requests for automated decisions with legal or similarly significant effects.

Visitor statistics
We measure aggregate traffic on the production website using Plausible. With visitor consent we also use Google Analytics, provided by Google; processing may take place outside the European Economic Area. Google Analytics is not loaded before consent and does not receive the name, phone number, email or note from the form. You can change your choice on the Cookies page.
